Point 1: Corporate titleProvide the name of the corporation. If you wish to verify the exact name of the organization, you can locate it using the Corporations Canada online database.
Point 2: Corporation numberSpecify the corporation number. You will find the corporation number on the certificate of incorporation, amalgamation, or continuance. It is also accessible through the Corporations Canada online database.
Point 3: Status of the corporationA bankrupt or insolvent corporation may only apply for termination three years after the trustee in bankruptcy or receiver has been discharged.

ImportantIn order to apply for termination under 4D – section 221 – the corporation must have requested and obtained a Certificate of Intent to Terminate from Corporations Canada. For more information on applying for a Certificate of Intent to Terminate, please refer to the instructions for Form 4019: Statement of Intent to Terminate.
Point 5: Custody of corporate documents and recordsAll terminating corporations must designate an individual who will be responsible for maintaining custody of the corporate records for six years after the termination date. If there are any changes to the person or their contact information within the six-year period following the termination, Corporations Canada must be notified in writing.
Point 6: DeclarationThis form must be signed by either a director or an authorized officer of the corporation. Please print the name and indicate the telephone number of the signatory.
